    (One Year: 1st April to 31st March Each Year)
  1. Membership Request on Company’s Letterhead be addressed to Secretary General, PVMA
  2. Copy of Memorandum & Article of Firm
  3. Plant & Machinery details on Company’s Letterhead duly singed by Authorized Person
  4. Details of Key Persons, Technicians and Labors employed on company’s letterhead duly singed by Authorized Person
  5. Copy of CNIC of Chief Executive/MD
  6. Copy of PSQCA Certificate (if applied, then, please submit copy of receipt)
  7. Pay Order /DD of 150,000/- One hundred fifty thousand only in favor of PVMA (Rs. 150,000/- as an Entrance Fee and Rs. 150,000/-) as Subscription Fee
  8. Copy of Sales Tax Registration (STRN)
  9. Copy of National Tax Certificate (NTN)
  10. Copy of last paid utility bills
  11. Mobile & Email (Chief Executive)
